Requirements
Candidates must have experience delivering or coordinating Enhanced Care Management services within behavioral health, community health, or social service systems. Applicants should demonstrate skill in leading multidisciplinary teams, supporting Community Health Workers and Care Coordinators, and overseeing care planning, outreach, documentation, and engagement across multiple rural and frontier counties. Strong knowledge of DHCS, Medi-Cal requirements, and ECM/CS service standards is essential. Reliable transportation, a valid driver’s license, current registration and insurance, and a safe driving record are required due to regional travel throughout Kern, Inyo, Mono, and Tulare counties. Lived experience relevant to the communities served is strongly valued. Candidates must be able to navigate complex care systems, support staff development, maintain high-quality service delivery, and hold professional liability coverage if their discipline requires it.
