About the Role
Requirements
Candidates must demonstrate lived experience that aligns with the role—such as personal or family experience with behavioral health, recovery, caregiving, housing instability, reentry, or navigating public systems—along with the ability to build trust in rural and frontier communities. Applicants must be willing and able to serve their designated region within Kern, Inyo, Mono, or Tulare counties, including remote and outlying areas. Reliable transportation, a valid driver’s license, current registration and insurance, and a safe driving record are required. Candidates must either hold a California Community Health Worker certification upon hire or be able to obtain the CHW credential within 18 months of employment. Familiarity with ECM and Community Supports services is preferred, and a commitment to culturally grounded, relationship-based care is essential.
About the Company
River Sage Revival is a rural-rooted, community-driven organization dedicated to expanding access to meaningful behavioral health support across Kern, Inyo, and Mono counties. We walk alongside individuals and families through Enhanced Care Management and Community Supports, meeting people where they live, heal, work, and gather. Our work is shaped by the land, the river, and the belief that rural communities deserve the same quality of care, dignity, and opportunity as anywhere else. We build programs that strengthen belonging, reduce barriers, and restore the wellbeing of our neighbors—one relationship, one service, and one community at a time.
