Care Coordinator Requirements

Candidates must have experience in care coordination, case management, or community-based behavioral health services. Strong knowledge of Enhanced Care Management and Community Supports is preferred, along with the ability to navigate Medi-Cal systems, coordinate across providers, and support clients with complex needs. Applicants must be comfortable working in rural and frontier settings across Kern, Inyo, Mono, and Tulare counties, including meeting clients in their homes, communities, or local gathering spaces. Reliable transportation, a valid driver’s license, current registration and insurance, and a safe driving record are required. Lived experience that supports connection, trust-building, and cultural humility is strongly valued. Candidates should be skilled in communication, documentation, and problem-solving and be able to work collaboratively within a multidisciplinary team while maintaining confidentiality and professional integrity.
