Minimum two years of experience in community outreach, community health, social services, case management, or a related field. Equivalent combinations of education, lived experience, and transferable skills will be considered in place of formal work history. If you have spent time connecting people in your community to resources, building relationships across organizations, or helping others navigate systems, we want to hear from you. Valid California driver's license and reliable personal transportation required.
